dc.description.abstract DHIS2 is the most widely used open-source e-HMIS system that suite to provide flexibility, customization of data elements, data entry forms, validation rules, indicators, and reports to create a fully-fledged system for data management.However, evidence shows that data quality is poor and is not used especially at lower levels of health care for program decisions in Ethiopia, and it remains a major challenge and its implementation status has not been evaluated yet.
