Abstract:
Abstract
Background: Diabetic ketoacidosis (DKA) is a life-threatening complication of diabetes mellitus. This study is aimed at determining recurrence and associated factors of DKA in diabetic children.
Objectives: To determine recurrence of diabetic ketoacidosis and associated factors in diabetic children admitted to Federal Police Force General Hospital Paediatric Ward from January 1, 2020 to January 1, 2024.
Methods: Institution based retrospective, cross-sectional study was conducted on children with diabetes admitted to Federal Police Force General Hospital paediatrics ward. A structured questionnaire was used to collect data from charts and phone call interview. Binary logistic regressions were used to identify factors associated with recurrent DKA. Variables with P<0.2 on bivariate analysis were entered on multivariate analysis to identify independently associated factors with recurrent DKA and the final model significance was set at p<0. 05.
Results: Of the 125 paediatric patients with DKA, 38 (30.4%) were recurrent cases whereas 87 (69.6%) were newly diagnosed cases. Factors associated with recurrence of DKA in this study were mothers' educational status[AOR = 6.1, 95% CI: 1.1–38.5, p-value = 0.004], average HbA1c level [AOR = 9.1, 95% CI: 1.1–72.9, p-value=0.036], precipitating factor of DKA [AOR = 0.01; 95% CI: 0.002-0.12; p-value = 0.001] and follow-up frequency [AOR = 0.2, 95% CI: 0.05–0.92, p-value = 0.038].
Conclusions and Recommendations: Low maternal educational level, high average HbA1c short duration of appointment, and precipitating factors increased the likelihood of recurrence of DKA hence, identifying these factors and introducing appropriate interventions may reduce the incidence of recurrent DKA. Further studies with large sample size are recommended.
